Once you choose a therapist and select a potential time, you’ll see a brief informational form to fill out. Although not a mandatory box, please enter in the “Message to Practice” what symptoms you are currently experiencing or issues you would like help resolving. Failure to do so may result in your appointment being canceled. Each of our therapists has a specialty, per their bios, and if they do not feel they are a good fit for you, they may not accept your appointment request. You are always welcome to call or email the office if you have questions.

Once your form is submitted, your therapist will receive your request and choose to approve your appointment time. You will receive an email with login instructions to the portal and a separate email confirming your appointment. If you do not receive a confirmation email within 2-3 business days, your appointment request was not accepted. Again, do not hesitate to call the office if you are unsure.

Your therapist will send you electronic documents to fill out prior to your appointment regarding insurance and consent forms. Please complete these forms in a timely manner to confirm your attendance at the intake appointment.
